How much premium do I have to pay?
  • Participating members have a choice to select a plan with a minimum monthly premium from as little as USD5 to a maximum of USD20. The recommended total monthly premium contribution which includes the participation member, their spouse and their children, if applicable, should not be more than 10% of the participating member's monthly gross income to ensure the plan selected reflects their needs and that their family's financial well-being comes first. 
  • The minimum monthly premium is USD5.
  • This product is applicable to Life Assured with standard risk only. Life Assured with sub-standard risk will not be considered.
  • The premium will be deducted monthly by the employee’s HR, Finance Department or the member’s organization until the participating member is 60 years old or when they resign, or by the membership termination date, whichever comes first.
  • Grace period: HR or the member’s organization are given 30 days of grace period after the due date to make a premium payment directly to Forte​ Life.

 

What is the coverage?

Forte Life's Comprehensive Group Insurance provides participating members with the following coverage and benefits:

Death benefit due to any cause – Lump sum payment equal to the Sum Assured.
Death benefit due to accidental cause – Lump sum payment equal to the Sum Assured which is equal to 200% of the death due to any cause.
Total Permanent Disablement (TPD) due to illness or accident – lump sum payment equal to the Sum Assured.
Funeral expenses at USD1,000 flat for all plans due to any cause of death, except for USD5 plans, which will receive USD500.
Accumulated Cash Benefit (ACB) – The monthly premium collected shall be deducted against risk charges and all other applicable insurance expenses. Any surplus amount will be deposited with Forte Life for accumulation and growth at an annual rate which will be declared by the company on a yearly basis.
For those 50 and above, all types of coverage and benefits will be adjusted to 75% of the selected Sum Assured. The monthly premium amount remains unchanged.

How to claim
  1. Notify Forte Life within 30 days of the loss.
  2. Submit required claim documents within 90 days.
  3. Our team will process and settle claims within 10 working days after all complete documents have been submitted.
